An Operation Locate Alert Notification has been issued for the following subject:

Prince Joshua Flomo

Black male age 25

5'5" 135 pounds with black hair and brown eyes.  

Flomo possibly has an altered mental status and walked away from Covenant Medical Center on 02-22-15 at about 8:50 a.m.  He was last seen in the area of Bontraeger Park in the 3400 block of West 9th Street and the area to the north and west of there.  He was wearing a medical gown and no shoes at the time he walked away.  If any information is known about the whereabouts of this subject, please contact Black Hawk Consolidated Dispatch at 319-291-2515 immediately.  Below is a photo of Flomo.
